MUNICIPAL SCHOLARSHIP PROGRAM FOR HIGH SCHOOL STUDENTS

Requirements:

  • Original and Photocopy of Form 138
  • Certificate of Indigency (from Barangay Hall)
  • Certificate of Good Moral Character
  • 2 pcs 1 X 1 picture
  • General average must be 83 and above

Steps:

  1. Get a Municipal Scholarship Program application form from the Administrator’s Office.
  2. Fill up the application form and submit to the Administrator’s Office together with the rest of the requirements. Make sure all the requirements are complete before submission to avoid delays in processing.
  3. Student’s grades will be evaluated if they are qualified for the scholarship program.
  4. The application form will be endorsed to MSWDO for assessment/evaluation. (During the evaluation, the applicant and his/her family may be visited by the evaluators.)
  5. List of approved students will be posted in the Municipal Bulletin Board and schools.
  6. Wait for the announcement for the releasing of certificates that will be posted in the Municipal Bulletin Board and schools.
  7. After the certificates are released, the announcement for the releasing of allowance will be posted in the Municipal Bulletin Board and schools.
  8. When claiming the allowance, remember to bring a photocopy of the following: Certificate of Recognition, grades (Form 138 or Class Card) and student ID.
